Job Description:
We are currently hiring a Nurse Practitioner to join our CloseKnit team.
CloseKnit is a virtual first advanced primary care practice that will be
opening our first employer group onsite location in Washington, DC and are
looking for an individual interested in developing a new model of primary
care delivery. We are looking for a knowledgeable, empathetic, team leader
willing to apply clinical skills in a flexible manner to deliver on the
quadruple aim in a value-based arrangement for adult employees and their
families. While providing both in person and virtual primary care services,
the nurse practitioner will also be working with the CloseKnit team to
optimize the care delivery strategy, implement technical tools to create
clinician and patient satisfaction and create best practices in delivering a
hybrid model of care to our employer-based patients.
This position
will be Monday - Friday onsite approximately 8:00 am - 4:00 pm. No nights
and No weekends.
ESSENTIAL FUNCTIONS:Primary Care Services
Maintain a panel of patients who receive high quality, attentive,
longitudinal in person and virtual primary care.
Provide comprehensive
primary care including diagnosis, treatment, documentation, and minor
procedures for acute, preventive and chronic patient needs.
Work with
both patients and their families.
Occupational Health Services
Provide acute care to employees.
Work with employer group human resources
and plan account leaders to recommend and deliver patient health and
wellness services.
Work with employees and leadership to connect with
CloseKnit team to resolve health care delivery issues.
Optimize Care
Delivery Strategy and Best Practices
Collaborate with CloseKnit care
team to improve patient processes.
Work with the team to apply continuous
improvement processes to improve quality and patient experience and lower
cost in an agile manner.
Advise the CloseKnit product and technical teams
on new resources that would benefit patients and the care deliver model.
Leadership
Oversee care team members working with patient panel.
Serve as a thought leader in the creation of a new, scalable employer group
primary care delivery model.
SUPERVISORY RESPONSIBILITY:
Position
does not have direct reports but is expected to assist in guiding and
mentoring less experienced staff. May lead a team of matrixed resources and
will collaborate and mentor other primary care providers CloseKnit will add
at a future date.
QUALIFICATIONS:Education Level: Master's
Degree
Experience: 5 years working as a Family Nurse Practitioner.
Licenses/Certifications:Nurse practitioner Licensure in
Washington DC and willing to obtain additional licenses.
Board-certified
by either ANCC or AANP.
Prescriptive authorization including DEA.
Preferred QualificationsLicensed in the State of Maryland
Virtual
care delivery experience
Knowledge of computer-based systems, as well as
medical records and point-of-care technology
Knowledge of disease
prevention and disease management
Experience with adult and pediatric
patients in a primary or urgent care setting
